Colorado’s Smart Meal™ Restaurant Program

The Colorado Department of Public Health and Environment (CDPHE) created the Smart Meal program to help customers identify, and eating establishments promote healthy meal options. The Smart Meal seal clearly identifies menu items that meet the designated healthy guidelines.

Sales Data Collection Form

Smart Meal Sales Data Collection Form

The Sales Data Collection Form is for restaurants to use to record sales data of the Smart Meal items. The form may be used as-is, or as a guideline for reporting the information requested. Ideally, the restaurant would collect sales data on a weekly (average) basis during the first year of implementation of the Smart Meal program.

Guidelines

  • Baseline data must be collected prior to the launch of the program.
    • If the qualifying Smart Meal items are modifications of existing menu items, then the baseline data would consist of an average of sales of the existing items for a minimum of 6 months prior to the launch of the Smart Meal items.
    • If new Smart Meal items have been created, then the baseline data would consist of the sales data from the most similar non-Smart Meal menu items to be used as a comparison.
  • Sales data to be collected (may be an average or % of overall sales) monthly by the restaurant for the first year of implementation.
  • Instructions: At the end of each month, please list all items labeled with the Smart Meal Seal. Include the baseline* and record the number of each item sold during that month.

  • Year:

Baseline refers to sales data for the month before the items are labeled with the Smart Meal Seal, if available. If the item is new at the time it is labeled with the Smart Meal Seal, note “new item” in this field.
